Pranamitra • Rural Health Mission

Healthcare shouldn't end when the camp ends.

Pranamitra is building continuity in rural healthcare — connecting medical camps, doctors, patient health records and community health insights through technology.

Healthcare professionals and community members at a rural health camp

Our mission

A medical camp can help someone today. But what happens tomorrow?

Across many rural communities, healthcare is episodic. A patient gets screened. A doctor provides advice. A camp ends. Months later, the same patient may meet another doctor with little or no accessible medical history. Pranamitra is working to create continuity between those moments.

Today

  1. Camp
  2. Consultation
  3. Records fragmented or lost
  4. Patient starts again

With Pranamitra

  1. Camp
  2. Digital health record
  3. Future consultation
  4. Health history continues

The problem

The missing layer in rural healthcare is continuity.

01

Fragmented medical history

Patient information may not remain accessible between healthcare interactions.

02

Limited continuity of care

Different camps and healthcare providers may operate without a shared understanding of the patient's previous history.

03

Invisible community health patterns

Without organised longitudinal information, broader disease patterns can remain difficult to identify.

04

Healthcare access beyond cities

Many communities remain geographically and operationally difficult for traditional healthcare systems to continuously reach.

How it works

From a single camp to a continuing health history.

01

Reach communities

Healthcare teams and doctors conduct medical camps directly within rural communities.

02

Screen & consult

Patients receive consultations, basic screening and appropriate diagnostic assessments.

03

Digitise

Relevant medical information is recorded digitally using the Pranamitra system.

04

Build continuity

Future healthcare interactions can contribute to the patient's continuing medical history.

05

Understand communities

Over time, anonymised and ethically governed health information can help reveal population-level health patterns.

The system flow

Human stories

Behind every record is a person.

FIELD STORY 01

A first step: knowing the numbers

A health worker checks a community member’s blood pressure during a field programme. A simple screening creates a moment to listen, assess and guide the next conversation about care.

Identity and medical details remain private.

Clinical examinations in progress during a community health programme

FIELD STORY 02

Care brought closer to the community

Clinical examinations take place within the community camp itself. Patients can speak directly with healthcare workers without the first step of travelling to a distant facility.

Identity and medical details remain private.

FIELD STORY 03

Diagnostics where they are needed

Healthcare workers carry out an ECG screening during an outdoor camp. Bringing diagnostic support into the field helps make a more informed consultation possible at the point of care.

Identity and medical details remain private.

Technology

Technology built around the patient journey.

The Pranamitra platform is being developed to help healthcare teams capture and organise patient medical information during rural healthcare initiatives. It creates the foundation for longitudinal health histories and, as the network grows, deeper understanding of population health.

Long term

Better public health decisions

As Pranamitra reaches more communities, longitudinal health information may help healthcare professionals understand how conditions such as diabetes, hypertension and cardiovascular disease differ across populations and regions.

The objective is not simply to collect data. The objective is to make healthcare knowledge more useful for the communities it came from.

All health information must be handled with appropriate consent, security, privacy and ethical governance.

Our vision

A connected rural health network.

  1. Village
  2. Cluster
  3. District
  4. Region
  5. State

Pranamitra aims to gradually expand its healthcare network while building the systems required to create continuity of care across rural communities.
